TriMix Titration Guide

The goal of TriMix therapy is an erection firm enough for satisfactory sexual activity while minimizing the risk of prolonged erections and other side effects. The ideal erection lasts approximately 30 to 60 minutes and resolves completely without intervention.

How to Titrate

Begin with the starting dose recommended by your prescribing provider. If the response is inadequate, increase the dose gradually with each subsequent injection until the lowest effective dose is identified.

  • Increase by 2 to 5 units (0.02 to 0.05 mL) at a time
  • Wait at least 24 hours between injections
  • Do not increase the dose more frequently than directed by your provider

Example Titration Schedule

Injection #110 units (0.1 mL)
Injection #215 units (0.15 mL)
Injection #320 units (0.2 mL)
Injection #425 units (0.25 mL)
Injection #530 units (0.3 mL)

Continue increasing gradually until satisfactory rigidity is achieved.

Frequency Limits

  • No more than one injection within a 24-hour period
  • No more than three injections per week
  • Alternate sides of the penis with each injection
  • Store TriMix refrigerated, and discard it after the period your pharmacy specifies (often about 30 days)

Reading Your Response

Ideal Response

  • Firm enough for penetration
  • Begins working within 5 to 20 minutes
  • Lasts approximately 30 to 60 minutes
  • Resolves completely on its own

Signs Too Low

  • Partial erection only
  • Inadequate rigidity for intercourse
  • Erection loses firmness quickly

Signs Too High

  • Erection persists beyond 2 hours
  • Significant penile discomfort
  • Excessive bruising or swelling

Priapism (Prolonged Erection) Action Plan

1

More than 2 hours

Consider taking pseudoephedrine if approved by your physician.

2

More than 3 hours

Contact your urology provider immediately.

3

4 hours or longer

Proceed to the nearest emergency department immediately. This is a urologic emergency.

Injection Technique Reminders

  • Inject at the 10 o'clock OR 2 o'clock position on the side of the penile shaft
  • NEVER inject at the 12 o'clock position — the nerves, arteries, and veins (the dorsal neurovascular bundle) run there
  • NEVER inject at the 6 o'clock position — the urethra runs along the underside there
  • Use a fine 29- or 30-gauge insulin syringe, inserted at a 90° angle. Avoid any visible veins.
  • Apply gentle pressure to the injection site for 2 to 3 minutes after injecting
  • Record your dose and response to assist with future adjustments
